Provided through OptumRx or Kaiser
If you enroll in one of the Alera Group national medical plans (through BCBS, UnitedHealthcare or Surest), you receive prescription drug coverage through OptumRx. If you choose a Kaiser plan (available in California only), you receive prescription drug coverage through Kaiser.

How Coverage Works
- Coverage is for in-network participating pharmacies only.
- Coverage for some medications may be subject to prior authorization, quantity limits or step therapy requirements.
- Prescription drug home delivery/mail order can save you time and money on medications you take on a regular basis.
Your Cost with Coverage
- Covered medications are divided into tiers, or cost-share levels, based on the coverage formulary (approved drug list). In general, the higher the tier, the higher the price you pay.
- In the Surest, POS and Kaiser plans, you are responsible for the applicable plan copays on covered prescription drugs.
- In the HDHPs, you must meet your overall plan deductible before the plan copays apply for prescription drugs. The deductible is only waived for specific preventive medications.
Preventive Medications
- Certain preventive prescription medications are offered at no cost to you, in all of the Alera Group medical plan options.
- In the HDHP options, certain preventive prescription medications are available for a copay before the deductible.
