How Premera Prescription Drug Coverage Works
- Coverage is for in-network participating pharmacies only.
- Coverage for some medications may be subject to prior authorization, quantity limits or step therapy requirements.
- If you need “maintenance medications” — prescription drugs you take on a regular basis for conditions such as high blood pressure, arthritis, or diabetes — prescription drug home delivery/mail order can save you time and money. You can receive up to a 90-day supply of medication at your home or other preferred location for less than the retail copay. There is no additional cost for delivery.
Your Cost
- Covered medications are divided into tiers, or cost-share levels, based on the Premera formulary (approved drug list). In general, the higher the tier, the higher the price you pay for the prescription medication.
- In the PPO plan, you are responsible for the applicable plan copays on covered prescription drugs.
- In the HDHP plans, you must meet your overall plan deductible before the plan copays apply for prescription drugs. The deductible is only waived for specific HDHP/HSA preventive medications.
Preventive Medications
- Certain preventive prescription medications are offered at no cost to you, in all of the Alera Group medical plan options.
- In the HDHP options, certain preventive prescription medications are available for a copay before the deductible.
