Medical Coverage
Find participating providers, view plan details, manage your claims, and access your electronic ID card. Surest uses the UnitedHealthcare Choice Plus Network.
To confirm your provider is in the Surest network (UnitedHealthcare Choice Plus network) and to estimate costs in the Surest Plan:
- Select Find a Provider below.
- Enter your email address and the access code Alera2025.
- Agree to the terms and conditions.
- If prompted, enter the Surest Verification Code emailed to you.
- Select whether you want your email address to be saved for future Surest communications/access.
- Enter your zip code/location and begin using the Find care search bar option.
- Only in-network providers will be shown when using this option. The costs associated with the services you are seeking will show and may vary based on the provider. This is part of the plan design and is expected.
- After you are a Surest plan member, you can log in to your account or use the app to search for in-network providers and cost estimates.
After electing coverage, you can create your account. Select LOG IN below and follow the prompts. To find participating providers on the pre-member site (FIND A PROVIDER), use access code Alera2024.
