You are eligible for Alera Group benefits if you are an active colleague working a minimum of 30 scheduled hours per week.
Your dependents are eligible if they are:
- Your legal spouse or domestic partner.
- Your and/or your domestic partner’s child(ren)* up to age 26.
- Your disabled child(ren) up to any age (if disabled prior to age 19).*
*Includes natural, step, legally adopted/or a child placed for adoption, or a child under your legal guardianship.
When you enroll a new dependent for the first time, you must submit verification documentation. Documentation can include a marriage certificate, birth certificate or adoption papers, a copy of a court document/order that grants you legal custody and either legal conservatorship, or legal guardianship, and other documents to prove your dependent’s eligibility.

About Domestic Partner Coverage
To enroll your domestic partner, you must submit a completed and notarized Domestic Partner Affidavit confirming your domestic partner qualifies as a dependent.
Under federal law, Alera Group’s contribution toward the cost of coverage for your domestic partner and their dependents is considered taxable income to you.
Domestic partner premiums will be deducted on a post-tax basis. You may wish to consult with a tax advisor for more information.
When Eligibility Ends / Termination of Coverage (COBRA)
If you or a covered dependent no longer meet the eligibility requirements, or if your Alera Group employment ends, your medical, dental, and vision coverage ends on the last day of the month in which you become ineligible. Life Insurance, Disability Insurance, Flexible Spending Accounts (FSAs), and all other benefits end on the day you become ineligible.
You may be eligible to elect COBRA for yourself and your eligible dependents for certain benefits. You may also be eligible to take some life coverages with you on a direct bill basis. Please refer to the Benefits Separation Summary for complete details.
You are responsible for informing the Alera Group Benefits Center within 30 days of your dependents becoming ineligible for benefits.
