Lifestyle Spending Account

The Lifestyle Spending Account (LSA) gives you flexible funds to support your wellness. After you complete an annual physical, you’ll receive $500 into a spending account in your name to use toward eligible products and services, such as gym memberships, meditation apps, and massage therapy. This benefit is only available to active colleagues enrolled in an Alera Group medical plan.

The Basics

How It Works

If you’re a colleague enrolled in an Alera Group medical plan and complete a qualified annual physical, Alera Group will provide a contribution of $500 to your LSA. These funds can be used on a variety of health, wellness, and lifestyle-related expenses for yourself.

You must be an active colleague on an Alera Group medical plan at the time of your physical and when funds are issued. Physicals completed before enrolling in an Alera Group medical plan are not eligible.

Spouses, domestic partners, and children are not eligible for this benefit. 

What’s Covered

Your LSA can be used for products and services that support your well-being. This includes:

  • Athletic accessories and clothing
  • Nutrition and fitness apps
  • Sleep tracker
  • Sports or home gym equipment

What You Pay

There’s no cost to you. Alera Group funds your LSA one time per year, and you submit claims for reimbursement. There is no debit card with the LSA.  

Note: The LSA is considered taxable income.

How the LSA Process Works

Schedule a preventive care visit during the year.

You must be an active colleague on an Alera Group medical plan at the time of your physical and when funds are issued. Physicals completed before enrolling in an Alera Group medical plan are not eligible.
Note: If you had an annual physical between July 1, 2025-December 31, 2025, that counts for 2026. The only time a lookback period will be considered is in 2026. Beginning in 2027, all physicals must be completed within the calendar year.

Enrolled in UHC, BCBS or Surest through Alera Group

No action is required by you. Your medical carrier will notify MyChoice Accounts that you received the physical.

OR

Enrolled in Kaiser or use concierge medicine through Alera Group

You need to complete a form, which can be found on the Reference Center of the Alera Group Benefits Center. Then upload that form to MyChoice Accounts.

Approximately 45 to 60 business days after your physical, Alera Group will deposit $500 into your LSA. You will then see the funds on the MyChoice portal.

Submit a claim(s) and upload receipt(s) to MyChoice Accounts for a well-being purchase or service.

The purchase of an item or service can exceed $500, but the maximum reimbursement is $500.

Set up direct deposit to a personal bank account in the MyChoice Accounts portal, and you will be reimbursed within 3-5 business days once your reimbursement request has been approved.

If you don’t set up direct deposit, you will receive a check mailed to your mailing address on file within 7-10 business days once your reimbursement request has been approved.

Timing for LSA funding may vary due to delays caused by provider or claims processing or holidays. This is only an example timeline. If you have questions or don't see your LSA funds after 90 business days from your claims processing, call the Alera Group Benefits Center at 888-850-1765. To receive the LSA funds, you must be an active colleague enrolled in an Alera Group medical plan at the time of LSA funding.

What Counts as a Qualified Annual Physical?

To receive LSA funds, you’ll need to complete a qualified annual physical. A well-woman visit with an obstetrician/gynecologist is eligible. These appointments must be preventive in nature — not diagnostic — and focused on your overall health and wellness.

When scheduling an appointment, you should ask your provider for a "preventive annual physical" and confirm they will bill using the appropriate CPT code.
