Manage

Your Alera Group medical plan gives you access to care, support, and prescriptions to help manage chronic conditions like diabetes, asthma, heart disease, autoimmune conditions, and more. You can:

  • See specialists and primary care providers within your plan’s network
  • Access ongoing treatment, preventive care, and condition-specific support
  • Get no-cost coverage for certain preventive medications under your plan

If you’re diagnosed with a health condition, a Blue Cross Blue Shield or UnitedHealthcare advocate (depending on your medical plan) will reach out to you to offer personalized support.

Log in to your medical plan portal or call the number on the back of your medical ID card to learn more about your coverage.

Navigating the health care system is complicated and it helps to have a guide. Included Health can help you understand your medical plan benefits, find top in-network doctors, schedule appointments, submit claims, and review bills.

If you're enrolled in a UnitedHealthcare or Blue Cross Blue Shield medical plan, you can use Garner Health to find the best in-network doctors. Garner highlights providers with top patient satisfaction, the latest medical practices, and the best outcomes. When you choose a recommended doctor, you may also qualify for reimbursement on some of your out-of-pocket costs.

If you’re unable to work due to your illness or injury, you may be eligible for a leave of absence or disability through Alera Group:

Depending on your situation, you may be eligible for Family and Medical Leave Act (FMLA), Paid Family Leave, or a Personal Leave of Absence.

Short-Term Disability (STD) provides income replacement for up to 12 weeks. If your STD benefits run out before you’re able to return to work, Long-Term Disability (LTD) will begin providing income replacement.

If you need to request a leave of absence or file a disability claim, log in to Unum’s Total Leave platform.
